Two forces were shaping healthcare.
Before the 1980s, healthcare was primarily shaped by two foundational forces:
The unequal distribution of information
The unequal distribution of solutions.
Physicians were the sole gatekeepers of medical information, decisions were made without patient input, and care solutions were constrained by where you lived, how much money you had, or even what language you spoke. Healthcare was built on closed systems—care was done to you, not with you.
But that began to change. Starting in the 1980s, the rise of patient autonomy, informed consent, and more transparent communication cracked open the gates. These changes were even incorporated into the updated Hippocratic Oath. The shift from passive patient to empowered participant was underway. Then the Internet, social media, and AI supercharged that momentum. By 2023, 83% of global citizens actively sought more control over their healthcare decisions.

This human behaviour is Healthusiasm at its purest: People don’t only want to be cured anymore; they want to be well, happy, and in control. Healthusiasm reframes good health as the most essential human need that influences nearly every aspect of our lives. Health is the most valuable human asset, influencing their care expectations, food choices, travel habits, digital behaviours, and more. Therefore, healthcare is no longer a siloed service—it’s a transversal value. In the Transformation Economy, people no longer want passive health experiences. They seek information and solutions supporting their ongoing transformation—vitality, longevity, and self-actualisation.
(Un)equal distribution of information
Today, we are witnessing a dramatic shift in how information and, to a certain extent, solutions are distributed—thanks to the rise of the Internet and social media. These technologies have accelerated the equal distribution of information by making it searchable, shareable, and socially validated. People no longer wait to be informed—they seek, they share, and they act. And that behavior is now being amplified and radically transformed by AI.
Physicians, too, have been transformed by technology. The Internet connected them to up-to-date guidelines, global expertise, and real-time case discussions. Medscape extended medical education from periodic conferences to on-demand and always-on platforms. Communities like Figure1 and Sermo have turned clinical isolation into digital collaboration. This technological evolution has reduced disparities in access to knowledge across geography and institutions, empowering physicians from rural settings to urban hospitals alike. No longer are only elite institutions the monopolic hubs of medical expertise.

Artificial Intelligence is taking this revolution even further now. AI is no longer a tool; in some areas, it is already outperforming its human counterparts. Google's MedPaLM has already demonstrated the ability to surpass physicians on complex medical queries. Models like Chatgpt-4o have aced licensing exams with scores higher than many medical professionals. I'm sure you've read a ton of articles on this subject. AI is accelerating the democratisation of information. But more importantly, it’s slowly redefining (medical) knowledge and expertise, making doctors worried.

(Un)equal distribution of solutions
Today’s runners have unprecedented access to advanced solutions with real-time information, covering heart rate variability, gait analysis, oxygen levels, nutritional optimisation, and more. These solutions were previously not widely available. But now, they empower every runner to experiment, optimise, and ultimately discover a "scientific truth" tailored to their bodies and performance goals (read blogpost on Personal Science). As AI is being integrated into these existing tools today, it is accelerating the impact of these solutions even more, creating deeply personalised pathways to health optimisation.
However, this is only the beginning. One of the most profound shifts is the still to come: the swarms of AI agents —specialised, interoperable assistants that support specific healthcare functions. Patients are soon building their own swarm of AI agents who will be acting as health concierges, offering behavioural nudges, dietary guidance, sleep optimisation, and micro-level diagnostics. The expected emergence of Virtual AI Doctors from companies like Apple, Ping An Good Doctor, and Baidu will mark that pivotal moment in the near future. (read blogpost on Virtual AI Doctors) These tools can analyse symptoms, suggest interventions, and provide monitoring at a global scale. The implications are massive: more accurate diagnoses, faster decisions, and dramatically reduced barriers to care. Individuals are no longer just passive care recipients; they are curating their own real-time, data-driven health journeys. AI finally makes hyper-personalised health management scalable. It meets people where they are—emotionally, physically, and contextually.

But the transformation doesn’t stop there. On the physician side, agents like Dr. Scout (the diagnostic detective), Dr. Architect (the treatment planner), and Nurse Pulse (the monitor) are redefining how care is delivered. Supporting agents like the Translator, Clerkbot, and Quality Maven ensure that communication is clear, compliance is met, and bias is audited. These agents extend the physician’s reach, precision, and capacity by continuously collaborating together with them.

Soon, these AI agents on both sides of the care spectrum—physician and patient—will begin to interact directly. An AI-Patient will communicate health data, preferences, and contextual insights to one or more AI-Physicians, enabling more precise, timely, and personalised care recommendations. This direct, virtual interface between patient- and doctor-side AI agents marks a new era of synchronised, real-time, data-driven health management. Expertise is no longer about title or authority. It’s about relevance, transparency, and shared decision-making. In this way, AI doesn’t just rebalance power; it rewires health management and redefines the very structure of care. Static care models will feel broken in a world where real-time value is expected.
Two NEW forces shape healthcare.
At its core, AI is directly addressing—and dismantling—the two historical forces that shaped healthcare for millennia:
The unequal distribution of information
The unequal distribution of solutions.
While the Internet and social media laid the foundation for more equal access to information, AI takes this several steps further—radically democratising information into actionable knowledge. For the first time, both patients and professionals can access expert-level insights on demand.
Meanwhile, AI is now beginning to equalise access to solutions as well. From diagnosis and treatment recommendations to personalised lifestyle support and continuous monitoring, AI embeds healthcare directly into people's lives—regardless of geography, income, or institutional affiliation.
These two NEW forces—equal distribution of information and equal distribution of solutions—are accelerating at an unprecedented pace because of AI. As a result, the entire structure and rhythm of healthcare are being redefined. Together, these forces are not simply changing healthcare. They are reshaping what it means to live well in the 21st century.
This is not just a technological shift.
This is Healthusiasm—one of the most critical macro trends today.
